If your trade certificate (or a certificate of competency) has been lost or destroyed (for example in a house fire or flood), you may apply for a copy or duplicate.
Apply for a duplicate certificate / extract
Complete the application form and email it to dsd.tamfax@sa.gov.au or send it to the Department of State Development, GPO Box 320, Adelaide SA 5001. You must include a statutory declaration with an application for a duplicate certificate, outlining the reason for the loss or destruction of the original certificate. Your duplicate certificate will be mailed to you, usually within 3-4 weeks.
Note that certificates were first issued in the middle of 1967, so duplicates cannot be issued for apprenticeships completed before that date, although we may be able to provide an extract of your record.
Duplicate trade certificates can only be issued to former apprentices. Trainees can apply for an extract of their record using the extract application form above.
Regulation and Contract Management can only issue duplicate trade certificates and extracts for training contracts undertaken in South Australia. If you undertook your training elsewhere in Australia, contact the relevant State Training Authority.
