Prepare to apply to deliver FSK Foundation Skills

This online form is a mix of free-form text boxes and areas where you will be asked to upload documents. There is no save function, so once commenced you need to submit the form or start again.

If your application to add courses is successful, a variation notice will be sent to you by email including your new Schedule of Courses in accordance with clause 14 of the Skills for All Contract. You must sign and return the varied Schedule of Courses to the Minister.

The effective date of your new schedule of courses will be the day the signed contract variation is returned to the Minister (Skills SA).
